Traumatic Injuries to the Diaphragm

Abstract
|
|
|
A 13 1/2-year retrospective review of 40 patients with diaphragmatic injuries revelaed 24 blunt and 16 penetrating injuries.
In the blunt trauma group, 19 were left-sided and 10 were on the right side. Preoperative diagnosis was made in 18 patients. Delayed diagnosis(greater than 24 hours) during life occurred in 10 patients. Chest X-ray was diagnostic or highly
suggestive in
21 patients. All but 4 patients in this group had associated intraabdominal and or extraabdominal injuries. Transabdominal repair was performed in 8. Three deaths occurred in the blunt groups.
The penetrating diaphragmtic wound consist of 10 left sided and 6 right sided. Preoperative diagnosis occurred in only 6 patients. The penerating wound, hypotension or peritoneal signs dictated operation in the remaining 9 patients. Two deaths
occurred
from hypovolemic shock.
Blunt and penetrating diaphragmatic injuries remain a diagnostic challenge, associated injuries and delayed diagnosis determine the outcome.
